Discordant aortic stenosis can be challenging, particularly when a small aortic valve area is accompanied by a lower-than-expected gradient despite preserved ejection fraction and stroke volume. In this case-based session, Tim Ogle explores the often-overlooked influence of prolonged left ventricular ejection time on transvalvular flow and Doppler-derived gradients. Learn why stroke volume and flow rate are not the same, how prolonged ejection time can contribute to AVA–gradient discordance, and how careful measurement and assessment of flow can help bring the pieces together in challenging aortic stenosis studies.

Assessing aortic stenosis becomes challenging when the valve morphology, valve area, and Doppler gradients don’t tell the same story. In this case-based lesson, Tony Forshaw works through a complex study featuring severe right ventricular dilatation and dysfunction, an underfilled left ventricle, markedly reduced stroke volume, and a low transvalvular gradient despite findings suggestive of severe aortic stenosis.
